Assignment 2: Logic Model
The information below explains how to create a logic model using a table format. A logic model is a clear and simple way to show how a program or project is expected to work. It uses different categories to show how the parts of the project are connected and follow a logical order.

There is no single “correct” way to draw a logic model—it depends on your goals. For big projects with different stages, you might need more than one model. Still, most logic models include the same basic parts: inputs, outputs, and outcomes.
The template below combines elements from several well-known models, including those from the Kellogg Foundation, United Way, and the University of Wisconsin Extension Service. You can find free resources, templates, and worksheets for building these models on their websites.

Inputs: These are the resources needed to carry out and evaluate the project, such as staff, facilities, equipment, time, and money.
Constraints: These are the things that might slow down or block the project, like barriers or challenges.
Activities: These are the actions you take using the resources to reach your goals.
Outputs: These are the immediate results of the activities, such as products, services, or events.
Outcomes: These are the changes that happen because of the project.
- Short-term outcomes: Early effects like increased knowledge or changes in how people do things.
- Long-term outcomes: Bigger changes, such as shifts in behavior, culture, or regular practices.
- Impact: The overall effect the project has on the people or community it serves.
Assumptions: Things you believe to be true or take for granted—such as why the project matters or why it will work.
External Factors: Outside influences that can affect the project, like the economy, available resources, or social issues.

- Prepare a logic model based on your project. Create a table with headings that include: inputs, constraints, activities, short term (outcomes), long term (outcomes), and impact. Below the table, note Assumptions and External Factors.
- Upload your logic model to the Canvas drop box.
