Assignment Overview:

In this course, the Individual Success Plan (ISP) assignment is a collaborative effort between you and the course faculty. Its purpose is to create a plan that ensures successful completion of specific requirements for your program. These requirements include:

  • Completing the required practice immersion hours.
  • Fulfilling program competencies.
  • Engaging in work associated with the completion of your Direct Practice Improvement (DPI) Project.

Preconference Meeting:

For certain courses, such as DNP-815A, 840A, 830A, 955A, 960A, and 965A, a preconference meeting is required. This meeting involves you and your preceptor/mentor reviewing course- and learner-specific learning objectives, roles, responsibilities, and expectations during the practice immersion experience. Faculty and preceptor/mentor information will be shared, and any requested course or program information will be provided by you. During the meeting, you’ll discuss course and practice immersion goals, including potential projects. The faculty will review the submitted document and approve proposed projects. Additionally, an introductory email will be sent to your preceptor/mentor.

General Requirements:

Follow these guidelines for successful completion of the assignment:

  • Find and download Individual Success Plan (ISP) and Direct Practice Improvement (DPI) timeline documents in the DNP PI Workspace.
  • Review the DNP Program Milestones document in the DNP PI Workspace to identify applicable domains, competencies, and milestones for this course.
  • Plan your practice immersion experiences to address each competency, specifying the hours you’ll dedicate to meet your goals. Apply concepts from core courses and integrate scholarly readings for case reports.
  • Use the ISP to develop a personal plan, showcasing major milestones and deliverables in the timeline.
  • Within the ISP, specify deliverables such as DNP practice immersion contracts, comprehensive logs of practice hours, evaluations, updated CV, scholarly activities, competency self-assessment, reflective journal, course goals, and faculty/preceptor approvals.
  • While an ISP is required, a mentor signature is not needed for DNP-801A, 805A, or DNP-810A. However, preconference and postconference/evaluation meetings with preceptors/mentors are mandatory for all DNP courses from DNP-815A onward. These meetings may be face-to-face or virtual, and a signed document must be uploaded by the end of Topic 1.
  • Discuss course- and learner-specific objectives during each conference, as you are responsible for completing all mandated program experiences.
  • Use APA style for your writing assignments, with the APA Style Guide available in the Student Success Center.
  • Review the rubric before starting the assignment to understand expectations for successful completion.
  • While not required for LopesWrite, submit the assignment using the digital classroom’s assignment dropbox. Additionally, upload the deliverable to the Learner Dissertation Page (LDP) in the DNP PI Workspace for future reference.

Follow these steps for your course:

  • Identify Deliverables:
    • Figure out the specific things you need to complete for this course. Discuss this with your faculty, and keep them updated if there are any changes to your plan.
  • List Challenges:
    • Think about the challenges you might face during your practice immersion hours and meeting competency requirements. Jot them down. How could you overcome these challenges?
  • Preconference Evaluation:
    • Before certain courses, like DNP-801, there’s a meeting. Make sure to update your faculty or chair with any changes to your Individual Success Plan (ISP).
  • Contact Information:
    • Fill out the Contact Information table at the start of the ISP resource. Sign it and put the date when you finish.
  • Read ISP Document:
    • Go through the ISP document, paying attention to Learner Expectations, Derivation of the ISP, and Instructions for Completing the ISP.
  • Complete ISP:
    • Follow the instructions in the document and complete the Individual Success Plan (ISP).

Your ISP outlines what you need to accomplish for success in your current course and the entire program.
