Type 2 diabetes mellitus (T2DM) is a globally prevalent issue, common in older adults. About 90 to 95% of all diabetes conditions in the U.S. are type 2 (Cleveland, 2025). Managing this condition is a public health priority. In addition to pharmacological interventions, self-management practices also play a significant role in type 2 diabetes management. Nurses play a significant role in helping patients control and manage this condition. Diabetes Self-Management Education (DSME), when led by trained nurses who are skilled in both clinical and behavioral education, is one of the most effective evidence-based interventions.
Several studies underscore the effectiveness of nurse-led diabetes self-management education. Azami et al., 2018 asserts that nurse-led DSME programs are associated with improved lifestyle, clinical outcomes and psychosocial outcomes. Patients significantly improve HbA1c levels, glycemic control ability, blood pressure, weight, social support and self management practices through DSMES training (Dailah, 2024).
Although DSMES training is associated with improved type 2 diabetes outcomes, several issues bar its effectiveness. It is important to assess the effectiveness of nurse led DSMES interventions for managing type 2 diabetes in older adults. This project will utilize a tailored DSMES training program to improve the type 2 diabetes self management outcomes in older adult patients.
